These trends coincide with shifting attitudes toward childhood vaccination likely fueled in part by vaccine misinformation. The past few years have seen more <a href=\"https:\/\/www.kff.org\/other-health\/measles-elimination-status-what-it-is-and-how-the-u-s-could-lose-it\/\">skepticism<\/a> and <a href=\"https:\/\/www.kff.org\/health-information-trust\/kff-tracking-poll-on-health-information-and-trust-covid-19-vaccine-update\/\">confusion<\/a> among the public about the safety and effectiveness of&nbsp;vaccines, a&nbsp;<a href=\"https:\/\/www.kff.org\/from-drew-altman\/the-problem-isnt-trust-in-vaccines-its-that-people-dont-know-who-to-trust\/\">decline in trust<\/a>&nbsp;of health authorities, and a growing <a href=\"https:\/\/www.kff.org\/health-information-trust\/kff-tracking-poll-on-health-information-and-trust-vaccine-safety-and-trust\/\">partisan<\/a> <a href=\"https:\/\/www.kff.org\/from-drew-altman\/the-problem-isnt-trust-in-vaccines-its-that-people-dont-know-who-to-trust\/\">divide<\/a>. Shifts in vaccine attitudes are reflected in recent state level policy changes, with state lawmakers introducing <a href=\"https:\/\/www.ncsl.org\/state-legislatures-news\/details\/vaccine-policy-remains-a-topic-of-interest-for-state-legislatures\">more than 2,500<\/a> vaccine-related bills since 2021, with almost half targeting vaccine requirements. In addition, Florida officials recently <a href=\"https:\/\/www.kff.org\/quick-take\/most-parents-nationally-and-in-florida-want-schools-to-require-vaccines\/\">announced<\/a> plans to eliminate all school vaccination requirements. Despite these changes, recent KFF polling found that public confidence in the safety of routine vaccines like MMR remains <a href=\"https:\/\/www.kff.org\/health-information-trust\/kff-tracking-poll-on-health-information-and-trust-the-publics-views-on-measles-outbreaks-and-misinformation\/\">high<\/a> and about <a href=\"https:\/\/www.kff.org\/public-opinion\/kff-the-washington-post-survey-of-parents\/\">eight in ten<\/a> (81%) parents overall as well as large majorities of parents who identify as Democrats, independents, and Republicans support current state vaccine requirements, saying students should be required to be vaccinated against measles and polio to attend public schools with some exceptions. This policy watch examines recent state policy changes to school vaccine requirements and the extent to which they may impact vaccination trends.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>States&nbsp;and local jurisdictions, not the federal government, set vaccine <\/strong><a href=\"https:\/\/www.cdc.gov\/vaccines\/php\/requirements-laws\/state-vaccination-requirements.html\"><strong>requirements<\/strong><\/a><strong> for daycare and school entry.<\/strong> The federal government does, however, have a long-standing, evidence-based&nbsp;<a href=\"https:\/\/www.cdc.gov\/vaccines\/basics\/how-developed-approved.html\">system<\/a>&nbsp;for approving and recommending vaccines for the public, including the <a href=\"https:\/\/www.cdc.gov\/vaccines\/hcp\/imz-schedules\/child-adolescent-age.html\">childhood vaccination schedule<\/a>. The childhood vaccination schedule is <a href=\"https:\/\/www.cdc.gov\/vaccines-children\/schedules\/who-sets.html\">set<\/a> by the Centers for Disease Control and Prevention (CDC) based on recommendations from the Advisory Committee on Immunization Practices (ACIP). ACIP\u2019s recommendations are <a href=\"https:\/\/www.astho.org\/topic\/resource\/impact-of-acip-recommendations-on-state-law\/\">used<\/a> by <a href=\"https:\/\/www.ncsl.org\/health\/state-non-medical-exemptions-from-school-immunization-requirements\">many states<\/a> to develop school vaccine requirements. HHS Secretary Robert F. Kennedy Jr. (RFK Jr.), who has long record of opposing immunizations and <a href=\"https:\/\/apnews.com\/article\/rfk-kennedy-election-vaccines-2ccde2df146f57b5e8c26e8494f0a16a\">spreading<\/a> vaccine misinformation, has led recent <a href=\"https:\/\/www.kff.org\/other-health\/acip-cdc-and-insurance-coverage-of-vaccines-in-the-united-states\/\">efforts<\/a> to re-examine the federal childhood vaccine schedule, replace members of ACIP, and <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/08\/27\/nx-s1-5515503\/fda-covid-vaccines-restricted\">restrict<\/a> COVID-19 vaccines and mRNA vaccine research. In addition, the Trump administration recently released a <a href=\"https:\/\/www.whitehouse.gov\/wp-content\/uploads\/2025\/09\/The-MAHA-Strategy-WH.pdf\">report<\/a> that calls for a new vaccine framework that includes reevaluating the childhood vaccine schedule and addressing vaccine injuries.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>All states currently <\/strong><a href=\"https:\/\/www.kff.org\/other\/state-indicator\/state-vaccine-requirements-for-children\/?currentTimeframe=0&amp;sortModel=%7B%22colId%22:%22Location%22,%22sort%22:%22asc%22%7D\"><strong>require<\/strong><\/a><strong>&nbsp;children to be vaccinated against certain diseases in order to attend public schools.<\/strong> School vaccination requirements are an important <a href=\"https:\/\/www.healthaffairs.org\/doi\/10.1377\/hlthaff.2024.00378\">tool<\/a> for reducing the spread of diseases and increasing vaccination coverage rates. Required vaccinations across every state and D.C. <a href=\"https:\/\/www.kff.org\/state-health-policy-data\/state-indicator\/state-vaccine-requirements-for-children\/\">include<\/a> MMR, DTaP, polio, and varicella; some states also require hepatitis A, hepatitis B, meningococcal, and\/or HPV vaccines. At this time, <a href=\"https:\/\/www.immunize.org\/official-guidance\/state-policies\/vaccine-requirements\/covid-child-school-2024\/\">no states<\/a> require the COVID-19 vaccine for school entry. While it has yet to be enacted, Florida\u2019s plan to eliminate all school vaccination requirements would make them the first and only state to do so. However, as the <a href=\"https:\/\/www.washingtonpost.com\/health\/2025\/09\/04\/coronavirus-vaccines-mandates-state-divisions\/\">divide<\/a> between red and blue states on health policy grows, <a href=\"https:\/\/www.statnews.com\/2025\/09\/09\/school-vaccine-requirements-florida-red-states-public-health\/\">more<\/a> states may consider moving this direction.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>All states&nbsp;allow&nbsp;exemptions from school vaccination requirements for medical reasons and almost all states (47 including D.C.) allow exemptions for religious and\/or personal beliefs (Figure 1).<\/strong> This leaves four states (California, Connecticut, Maine, and New York) that only allow medical exemptions. Studies have shown that higher exemption rates from school vaccination requirements are&nbsp;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8982917\/\">associated<\/a>&nbsp;with lower vaccination coverage rates and&nbsp;<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4202987\/\">increased<\/a>&nbsp;risk for disease outbreaks. In the 2024-2025 school year, the share of children claiming an exemption from vaccination requirements from one or more vaccinations <a href=\"https:\/\/www.kff.org\/medicaid\/kindergarten-routine-vaccination-rates-continue-to-decline\/\">rose<\/a> to 3.6%, the highest national exemption rate to date, up from 2.5% in 2019-2020. Increases in non-medical exemptions&nbsp;accounted&nbsp;for the recent increases, with non-medical exemptions increasing from 2.2% to 3.4% over the period.&nbsp;Vaccination rates and exemption rates vary significantly by state, with the <a href=\"https:\/\/www.kff.org\/state-health-policy-data\/state-indicator\/vaccination-exemption-rates-among-kindergartners\/\">share of children claiming an exemption<\/a> from one or more vaccinations during the 2024-2025 school year ranging from 0.1% in California to 15.4% in Idaho.<\/p>\n\n\n\n<div class=\"datawrapper-embed block--datawrapper-embed \">\n\t\t\t\t\t<div style=\"min-height: px;\">\n\t\t\t<script\n\t\t\t\ttype=\"text\/javascript\"\n\t\t\t\tdefer\n\t\t\t\tsrc=\"https:\/\/datawrapper.dwcdn.net\/GjyN2\/embed.js?v=14\"\n\t\t\t>\n\t\t\t<\/script>\n\t\t<\/div>\n\t\t\t<img\n\t\t\tclass=\"datawrapper-embed__print-img\"\n\t\t\tsrc=\"https:\/\/datawrapper.dwcdn.net\/GjyN2\/full.png\"\n\t\t\talt=\"47 States Allow Exemptions From School Vaccination Requirements for Religious and\/or Personal Beliefs\"\n\t\t\/>\n<\/div>\n\n\n\n\n<p class=\"wp-block-paragraph\"><strong>At least 10 states so far this year have enacted or issued changes related to routine vaccine requirements for children (Table 1).<\/strong> In the years following the pandemic, states <a href=\"https:\/\/apnews.com\/article\/vaccines-whooping-cough-rfk-measles-exemptions-covid-27dae6f61505ef1953ca869a78c71942\">saw<\/a> an <a href=\"https:\/\/www.ncsl.org\/state-legislatures-news\/details\/vaccine-policy-remains-a-topic-of-interest-for-state-legislatures\">increase<\/a> in vaccine-related policy proposals. The pandemic spurred increased state legislative activity initially focused on state-level authority to <a href=\"https:\/\/nashp.org\/state-tracker\/states-address-school-vaccine-mandates-and-mask-mandates\/\">require<\/a> COVID-19 vaccines. However, over time, as the response to COVID-19 became more politicized, states began <a href=\"https:\/\/nashp.org\/state-tracker\/state-efforts-to-ban-or-enforce-covid-19-vaccine-mandates-and-passports\/\">limiting<\/a> COVID-19 vaccine mandates as well as <a href=\"https:\/\/nashp.org\/trends-in-vaccine-related-legislation-coming-out-of-2023-state-legislative-sessions\/\">focusing<\/a> more broadly on routine vaccination requirements (and exemptions to those requirements) in schools. Notably, <a href=\"https:\/\/www.ncsl.org\/health\/state-public-health-legislation-database\">most<\/a> of the vaccine-related bills introduced by legislators since the pandemic began have not passed, but ten states in the past year have enacted or issued policy changes related to school or child care vaccination requirements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Almost all states (nine out of 10 states with recent changes) made changes that could result in more students claiming a non-medical exemption, which could reduce vaccination rates. <\/strong>Many of the recent changes (described in Table 1) will make it easier for families with children in childcare settings or school to obtain a non-medical vaccine exemption. Notably, the governor of West Virginia signed an executive order allowing religious and personal belief exemptions in January 2025, though litigation is <a href=\"https:\/\/westvirginiawatch.com\/2025\/08\/22\/federal-office-says-west-virginia-should-allow-religious-exemptions-for-school-vaccine-requirements\/\">ongoing<\/a>. Prior to 2025, West Virginia was one of five states (now four states) that only allowed medical exemptions and had the <a href=\"https:\/\/www.cdc.gov\/mmwr\/volumes\/73\/wr\/mm7341a3.htm\">highest<\/a> vaccination rates and lowest exemption rates in the country during the 2023-2024 school year (the latest available data for the state). In addition, Idaho, the state with the <a href=\"https:\/\/www.kff.org\/medicaid\/kindergarten-routine-vaccination-rates-continue-to-decline\/\">lowest<\/a> vaccination rates and highest exemption rates during the 2024-2025 school year, transferred control of required vaccines to the legislature and restricted medical mandates (likely <a href=\"https:\/\/www.kivitv.com\/twin-falls\/does-idaho-medical-freedom-act-do-away-with-school-immunization-requirements\">weakening<\/a> school vaccine requirement enforcement). These changes, in addition to broader state efforts to <a href=\"https:\/\/stateline.org\/2025\/02\/28\/vaccination-rates-are-declining-they-might-get-worse-as-states-relax-rules\/\">scale back<\/a> immunization outreach and <a href=\"https:\/\/www.cnn.com\/2025\/02\/13\/health\/louisiana-mass-vaccination\">promotion<\/a>, changes to vaccine recommendations at the federal level, and <a href=\"https:\/\/www.kff.org\/policy-watch\/measles-elimination-status-what-it-is-and-how-the-u-s-could-lose-it\/\">reduced support<\/a>\u00a0from the federal government for state and local health departments, could further\u00a0<a href=\"https:\/\/www.texastribune.org\/2025\/08\/18\/texas-school-vaccination-budget-cuts\/\">increase<\/a> the number of exemptions requested and drive down vaccination rates among children. Florida\u2019s plan to <a href=\"https:\/\/www.nytimes.com\/2025\/09\/03\/us\/florida-vaccine-mandates.html\">eliminate<\/a> all school vaccination requirements goes beyond expanding exemptions and would mark a major shift in state vaccination requirements,\u00a0though the issue will have to be taken up by the state legislature.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>At the same time, one state has made a change that could maintain or increase children\u2019s routine vaccination rates. <\/strong>Colorado recently passed a law allowing the consideration of vaccine recommendations from outside groups like the American Academy of Pediatrics (AAP), not only ACIP, when developing school vaccine requirements. More states may move in this direction depending on the outcome of <a href=\"https:\/\/www.cdc.gov\/acip\/meetings\/index.html\">ACIP\u2019s upcoming meeting<\/a> on vaccine recommendations and <a href=\"https:\/\/www.washingtonpost.com\/health\/2025\/09\/08\/rfk-jr-new-vaccine-advisers\/\">potential<\/a> further changes by RFK Jr. to the ACIP panel. In addition, the AAP also recently reaffirmed their <a href=\"https:\/\/publications.aap.org\/aapnews\/news\/32619\/AAP-Nonmedical-exemptions-to-school-immunization?autologincheck=redirected\">support<\/a> for eliminating non-medical exemptions amid rising exemption rates, and some states, such as <a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/vaccinations-rise-states-religious-loopholes-exemptions-rcna212334\">Massachusetts<\/a> and <a href=\"https:\/\/www.hawaiipublicradio.org\/local-news\/2025-02-10\/hawaii-measure-non-medical-vaccine-exemptions-in-schools\">Hawaii<\/a>, are proposing eliminating non-medical exemptions, though these changes have not been enacted. While the appointment of RFK Jr. in early 2025 likely <a href=\"https:\/\/www.pbs.org\/newshour\/politics\/state-lawmakers-see-new-opportunities-to-pass-vaccine-exemptions-after-trumps-return\">spurred<\/a> additional efforts to loosen vaccine requirements in many states this year, other states are <a href=\"https:\/\/www.wired.com\/story\/states-are-moving-to-protect-access-to-vaccines\/\">working<\/a> to ensure vaccine access amid changes at the federal level.<\/p>\n\n\n\n<div class=\"datawrapper-embed block--datawrapper-embed \">\n\t\t\t\t\t<div style=\"min-height:
