This is the latest action by the second Trump administration reversing major Biden administration waiver priorities. 1115 <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/medicaid-section-1115-waivers-the-basics\/\">waivers<\/a>&nbsp;generally reflect priorities identified by states as well as&nbsp;<a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/medicaid-waiver-priorities-under-the-trump-and-biden-harris-administrations\/\">changing priorities<\/a>&nbsp;from one presidential administration to another.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Gaps in access to certain providers (e.g., psychiatrists, primary care providers, dentists, and long-term care providers) have been ongoing challenges in Medicaid. Access gaps may reflect system-wide problems, but may be exacerbated by <a href=\"https:\/\/www.kff.org\/state-category\/providers-service-use\/health-professional-shortage-areas\/\">provider\u00a0shortages<\/a>\u00a0in low-income communities, Medicaid\u2019s\u00a0lower physician\u00a0<a href=\"https:\/\/www.healthaffairs.org\/doi\/epdf\/10.1377\/hlthaff.2020.00611\">payment rates<\/a>,\u00a0and lower\u00a0Medicaid\u00a0<a href=\"https:\/\/www.macpac.gov\/wp-content\/uploads\/2021\/06\/Physician-Acceptance-of-New-Medicaid-Patients-Findings-from-the-National-Electronic-Health-Records-Survey.pdf\">physician<\/a>\u00a0<a href=\"https:\/\/www.healthaffairs.org\/do\/10.1377\/forefront.20190401.678690\/full\/\">participation<\/a>\u00a0compared with private insurance. <a href=\"https:\/\/www.kff.org\/mental-health\/issue-brief\/a-look-at-strategies-to-address-behavioral-health-workforce-shortages-findings-from-a-survey-of-state-medicaid-programs\/\">State strategies<\/a> to address workforce shortages can include <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/payment-rates-for-medicaid-home-care-states-responses-to-workforce-challenges\/\">increasing rates<\/a>, reducing provider administrative burden, extending the workforce, and incentivizing participation (such as through student loan repayment or retention bonuses). Improving coverage and access was a <a href=\"https:\/\/www.healthaffairs.org\/content\/forefront\/strategic-vision-medicaid-and-children-s-health-insurance-program-chip\">strategic priority<\/a> for the Biden administration, as was strengthening the <a href=\"https:\/\/bidenwhitehouse.archives.gov\/briefing-room\/statements-releases\/2023\/05\/18\/fact-sheet-biden-harris-administration-announces-new-actions-to-tackle-nations-mental-health-crisis\/\">mental health workforce<\/a>. Section 1115 waivers have been used as a strategy to invest in the Medicaid workforce and help address provider shortages. Changes to 1115 waiver policy and broader federal Medicaid <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/allocating-cbos-estimates-of-federal-medicaid-spending-reductions-across-the-states-enacted-reconciliation-package\/\">spending reductions<\/a> may limit states\u2019 abilities to pursue initiatives investing in the Medicaid workforce, as well as cause states to take actions such as <a href=\"https:\/\/ncnewsline.com\/2025\/08\/13\/north-carolina-cut-medicaid-rebase-by-3-percent-all-providers-october-sangvai\/\">reducing<\/a> Medicaid provider payment rates that could increase gaps in access.\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Since June 2022, five states (California, Massachusetts, New York, North Carolina, and Vermont) have received approval for waivers that invest in the Medicaid workforce. <\/strong><a href=\"https:\/\/www.mass.gov\/statewide-investments\">Some states<\/a> had used 1115 waivers prior to 2022 to expand workforce capacity, most notably through Delivery System Reform Incentive Payment <a href=\"https:\/\/www.macpac.gov\/wp-content\/uploads\/2018\/03\/Exploration-of-the-Evolving-Promise-of-DSRIP-and-Similar-Programs.pdf\">(DSRIP) waivers<\/a>. Approved workforce initiatives since 2022 have been focused on investments in qualified providers who make multi-year commitments to working with a patient panel with a minimum percentage of Medicaid and\/or uninsured patients. These initiatives include: student loan repayment programs for qualified providers (e.g., registered nurses, nurse practitioners, physician assistants, psychiatrists), recruitment and retention bonuses, training programs, and payments to support additional residency slots (Table 1). The workforce initiatives primarily target providers working in behavioral health, primary care, and long-term care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the approvals, <a href=\"https:\/\/www.medicaid.gov\/sites\/default\/files\/2024-01\/ny-medicaid-rdsgn-team-appvl-01092024.pdf\">CMS explained<\/a> that states \u201ccontinue to face health care provider shortages,\u201d which were further exacerbated by the COVID-19 public health emergency, and that these approvals were intended to increase the availability of providers who serve Medicaid beneficiaries. North Carolina, for example, <a href=\"https:\/\/www.medicaid.gov\/medicaid\/section-1115-demonstrations\/downloads\/nc-medicaid-reform-extns-req-pa.pdf\">reported<\/a> in its application that psychiatrists in North Carolina are only meeting <a href=\"https:\/\/www.kff.org\/statedata\/mental-health-and-substance-use-state-fact-sheets\/north-carolina\/\">13% of the need<\/a> in the state, and nearly a third of counties do not have any practicing psychologists. California <a href=\"https:\/\/www.medicaid.gov\/medicaid\/section-1115-demonstrations\/downloads\/ca-bh-connect-pa-10202023.pdf\">stakeholders<\/a> identified the behavioral workforce crisis as the most significant challenge to implementing new behavioral health services in the state. While there is limited implementation data on these workforce initiatives to date (as several waivers were only recently approved), Section 1115 demonstration waivers are subject to regular reporting, monitoring, and evaluation requirements that will provide evidence on the effectiveness of these types of interventions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Combined, four states have been approved for at least $2.7 billion in <em>total <\/em>expenditure authority (including both federal and state spending).<\/strong> (In Vermont\u2019s waiver, workforce initiatives are included as part of an umbrella \u201cInvestments Framework,\u201d so maximum funding amounts specific to workforce programs are not outlined.) These workforce initiatives are components of broader 1115 waivers. Waivers must be <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/medicaid-section-1115-waivers-the-basics\/\">budget neutral<\/a> (meaning federal costs under an 1115 waiver may not exceed what they would have been for that state without the waiver) and states must use waiver savings to offset expenditures on the approved 1115 workforce programs. &nbsp;In addition, <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/section-1115-waiver-watch-early-signs-point-to-new-directions-under-trump-administration\/\">CMS specified<\/a> that some states could access federal matching funds for spending on \u201cDesignated State Health Programs (DSHP),\u201d with states using \u201cfreed up\u201d state funds to help finance the state share of new workforce initiatives.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>In its <\/strong><a href=\"https:\/\/www.medicaid.gov\/resources-for-states\/downloads\/workforc-ltr-to-states.pdf\"><strong>recent letter<\/strong><\/a><strong>, CMS said it would <em>not <\/em>approve 1115 workforce initiatives going forward. <\/strong>Currently approved waivers will be allowed to \u201crun their course,\u201d but CMS does not anticipate extending the workforce initiatives when the waivers are due for renewal. Most of the approved workforce waivers are set to expire in 2027, although California and North Carolina\u2019s waivers are not due to expire until 2029 (Table 1). CMS is also unlikely to approve requests for new workforce initiatives, such as <a href=\"https:\/\/www.medicaid.gov\/medicaid\/section-1115-demonstrations\/downloads\/fl-hcwf-pa-05162025.pdf\">Florida\u2019s<\/a> May 2025 request for federal matching funds for workforce training and loan repayment programs for doctors, nurses, physician assistants, dentists, dental hygienists, and mental health practitioners who serve Medicaid recipients in medically underserved areas. In the letter phasing out workforce authority, CMS said it is prioritizing \u201cactions that demonstrate clear health benefits, cost savings, and strong accountability for federal spending.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>This CMS guidance is the latest action by the Trump administration undoing Biden administration 1115 waiver policies. <\/strong>CMS also\u00a0announced\u00a0in July that it does not anticipate extending existing or approving new section 1115 continuous eligibility waivers for adults <em>or<\/em> children enrolled in Medicaid coverage.Previous actions\u00a0from the Trump administration earlier this year also signaled efforts to curtail waivers related to social determinants of health (which also included efforts to bolster access and increase provider payment rates) and to limit waiver financing tools and flexibility.
