A placebo-controlled trial is a study that compares participants who receive an experimental vaccine to a control group that receives a placebo\u2014an inert substance\u2014to help isolate the effects of the vaccine from other factors. While many vaccines, including <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2034577\">COVID-19 vaccines<\/a>, were tested this way, the <a href=\"https:\/\/code-medical-ethics.ama-assn.org\/ethics-opinions\/ethical-use-placebo-controls-research\">American Medical Association (AMA)<\/a> and the <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4157320\/\">World Health Organization (WHO)<\/a> do not consider placebos ethical when withholding a known effective vaccine could expose participants to unnecessary risk. In such cases, researchers use other <a href=\"https:\/\/www.factcheck.org\/2025\/05\/hhs-advances-kennedys-old-placebo-vaccine-safety-claims\/\">rigorous methods<\/a>, such as comparing a new vaccine to an approved one to evaluate differences in safety, immune response, and effectiveness. The HHS announcement has led to questions about the safety of existing vaccines among those unfamiliar with how clinical trials are designed and why placebo use is not always appropriate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Online discussion about placebo-controlled trials spiked on May 1 and May 4, following news reports of HHS\u2019 new policy. Others misleadingly presented the policy change as evidence that past vaccine trials lacked proper oversight or were \u201ccorrupt.\u201d A widely shared post from a medical doctor and lawyer with over 900,000 followers called the update long overdue and said it would finally hold vaccine makers to higher safety standards. This framing has led to confusion about what qualifies as a \u201cnew vaccine,\u201d particularly in contrast to updated COVID-19 boosters. While a new vaccine may target either a novel or existing pathogen using new technology or formulations, COVID-19 boosters are updates to previously approved vaccines designed to target evolving strains of the same virus. Unlike brand-new vaccines, which undergo extensive clinical trials, updated COVID-19 boosters were <a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2022\/08\/18\/1117778748\/whats-behind-the-fdas-controversial-strategy-for-evaluating-new-covid-boosters\">previously<\/a> approved using existing safety data from the original vaccine, along with lab and animal studies showing that the updated shot produces a strong immune response to new variants. Experts <a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/updated-covid-vaccines-fall-may-jeopardy-rfk-jrs-new-rules-rcna204242\">warn<\/a> that the shift could delay access to boosters as new strains of viruses spread, while weakening trust in long-standing safety procedures.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Confusion about boosters began to grow after the FDA <a href=\"https:\/\/www.cbsnews.com\/news\/fda-covid-booster-shots-marty-makary\/\">questioned<\/a> data around boosters and <a href=\"https:\/\/www.cidrap.umn.edu\/covid-19\/more-fda-uncertainty-roils-full-approval-novavax-covid-vaccine\">delayed<\/a> full approval of Novavax\u2019s updated COVID-19 vaccine, <a href=\"https:\/\/x.com\/DrMakaryFDA\/status\/1916253910039429309\">calling<\/a> it a new product and requesting additional trial data. Novavax, a protein-based alternative to mRNA vaccines, has been authorized for <a href=\"https:\/\/www.fda.gov\/vaccines-blood-biologics\/coronavirus-covid-19-cber-regulated-biologics\/novavax-covid-19-vaccine-adjuvanted\">emergency use<\/a> since 2022 after a large <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2116185?query=featured_home\">placebo-controlled trial<\/a> showed high efficacy and few serious adverse effects. The updated version targets newer variants and functions as a booster for individuals who have previously been vaccinated. Prior to the FDA\u2019s <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMsb2506929?utm_source=substack&amp;utm_medium=email\">decision<\/a> to apply new product standards for all COVID-19 boosters for people under 65, some high-profile social media accounts reacted to the Novavax delay with concern about boosters. Extensive <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9917162\/\">research<\/a> and real-world data show that mRNA vaccines have a strong safety profile and offer protection against severe illness, hospitalization, and death from infectious diseases, like COVID-19, and potentially <a href=\"https:\/\/www.nature.com\/articles\/nrd.2017.243\">cancer<\/a>. But a group of legislators in Minnesota recently introduced a <a href=\"https:\/\/www.revisor.mn.gov\/bills\/text.php?number=HF3219&amp;type=bill&amp;version=0&amp;session_year=2025&amp;session_number=0\">bill<\/a> that would classify mRNA vaccines as \u201cweapons of mass destruction,\u201d with criminal penalties of up to 20 years in prison. Although unlikely to pass, the bill represents the growing reach of concern about mRNA technology. Similar measures have emerged in other states: <a href=\"https:\/\/www.legis.iowa.gov\/legislation\/BillBook?ba=SF360\">Iowa<\/a>, <a href=\"https:\/\/bills.legmt.gov\/#\/laws\/bill\/2\/LC1463?open_tab=bill\">Montana<\/a> and <a href=\"https:\/\/legislature.idaho.gov\/wp-content\/uploads\/sessioninfo\/2023\/legislation\/H0154.pdf\">Idaho<\/a> have considered proposals that would impose criminal penalties on providers.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Social media conversations continue to reflect unsubstantiated concern about mRNA vaccines, sometimes showing support for legislation aiming to ban them. One Texas doctor with over 500,000 followers, previously suspended for sharing false information about vaccines, said that Minnesota legislators were \u201clead[ing] the way\u201d with the proposal. Another widely shared post featured a video of a doctor with a revoked medical license recommending that viewers and their families \u201cnot get vaccinated, ever again, with an mRNA vaccine.\u201d These conflict with the broad <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9863643\/#sec6-ijms-24-01404\">scientific consensus<\/a> supporting mRNA vaccines and contribute to doubt about COVID-19 vaccines. <a href=\"https:\/\/www.fda.gov\/vaccines-blood-biologics\/safety-availability-biologics\/covid-19-vaccine-safety-surveillance\">According to the FDA<\/a>, serious adverse effects from the COVID-19 vaccines\u2014which are <a href=\"https:\/\/www.statista.com\/statistics\/1198516\/covid-19-vaccinations-administered-us-by-company\/\">predominantly mRNA-based<\/a> in the United States\u2014occur in fewer than 1 in 200,000 vaccinated individuals, and research has shown that COVID-19 vaccines have saved tens of millions of lives globally.<\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-group has-background is-layout-constrained wp-container-core-group-is-layout-17 wp-block-group-is-layout-constrained\" style=\"background-color:#e5e5e5;margin-top:var(--wp--preset--spacing--60);margin-bottom:var(--wp--preset--spacing--60);padding-top:var(--wp--preset--spacing--40);padding-right:var(--wp--preset--spacing--40);padding-bottom:var(--wp--preset--spacing--40);padding-left:var(--wp--preset--spacing--40)\">\n<p class=\"wp-block-paragraph\"><strong>Polling Insights:<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.kff.org\/health-information-and-trust\/poll-finding\/kff-tracking-poll-on-health-information-and-trust-vaccine-safety-and-trust\/\">New KFF polling<\/a> shows that mRNA technology is obscure to much of the public. About twice as many adults think vaccines that use mRNA technology are \u201cgenerally safe\u201d (32%) as say they are \u201cgenerally unsafe\u201d (16%), but about half (52%) report not knowing enough about this technology to say. In addition, nearly half of the public (45%) report having heard the false claim that mRNA vaccines can alter a person\u2019s DNA &#8212; a&nbsp;myth related to COVID-19 vaccines that <a href=\"https:\/\/www.kff.org\/coronavirus-covid-19\/poll-finding\/kff-covid-19-vaccine-monitor-april-2021\/\">began circulating<\/a> early in the pandemic. While just 3% think this false claim is \u201cdefinitely true\u201d and one quarter say it is \u201cdefinitely false\u201d (24%), most fall in the malleable middle, saying it is either \u201cprobably true\u201d (26%) or \u201cprobably false\u201d (45%). The 400-page <a href=\"https:\/\/opa.hhs.gov\/sites\/default\/files\/2025-05\/gender-dysphoria-report.pdf\">report<\/a>, commissioned by an <a href=\"https:\/\/www.federalregister.gov\/documents\/2025\/02\/03\/2025-02194\/protecting-children-from-chemical-and-surgical-mutilation\">executive order<\/a> aimed at <a href=\"https:\/\/www.kff.org\/policy-watch\/president-trumps-executive-order-on-gender-affirming-care-responses-by-providers-states-and-litigation\/\">limiting youth access to gender-affirming care<\/a>, states that it surveys existing literature on gender-affirming interventions for children and adolescents, concluding that \u201cthe evidence for benefit of pediatric medical transition is uncertain, while the evidence for harm is less uncertain.\u201d Major medical groups, including the <a href=\"https:\/\/publications.aap.org\/aapnews\/news\/32145\/AAP-speaks-out-against-HHS-report-on-gender\">American Academy of Pediatrics<\/a> and the <a href=\"https:\/\/updates.apaservices.org\/statement-on-access-to-treatment-for-transgender-gender-diverse-and-nonbinary-people#:~:text=Following%20the%20release%20of%20the,nonbinary%20children%2C%20teens%20and%20adults.\">American Psychological Association<\/a>, criticized the report and continue to support access to transition-related care based on other <a href=\"https:\/\/www.rand.org\/pubs\/research_briefs\/RBA3223-1.html\">reviews<\/a> and <a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2789423\">studies<\/a> that have found a positive impact of gender-affirming care on health. On the day the report was released, mentions of gender-affirming care spiked on social media. Some high-profile accounts, including a podcast host with almost five million followers, circulated quotes from the report\u2019s foreword that highlighted potential risks associated with gender-affirming care while emphasizing uncertainty about the benefits.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The report spurred other incorrect claims, including the false notion that surgical interventions are common among minors, but such procedures are exceedingly rare. One <a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2820437\">study<\/a> that looked at data from more than 22 million minors found that less than .01% of transgender and gender diverse minors ages 13 to 17 underwent gender-affirming surgery, and none under 12 received such care. The HHS report\u2019s framing of \u201cexploratory therapy,\u201d a practice that can include gender identity conversion efforts, drew attention online, but a <a href=\"https:\/\/www.kff.org\/mental-health\/issue-brief\/u-s-department-of-health-and-human-services-report-on-pediatric-gender-dysphoria-and-gender-conversion-efforts\/#:~:text=On%20May%201%2C%202025%2C%20the,Secretary%20to%20publish%20an%20evidence\">recent KFF brief<\/a> explains that major health associations have <a href=\"https:\/\/usjs.org\/wp-content\/uploads\/2023\/10\/USJS-Final-Version.pdf\">condemned<\/a> this therapy as harmful and unsupported by evidence. Social media posts also amplified claims that many youth regret care, but research shows regret is uncommon. While not all transgender people seek medical care, a <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8099405\/#sec23\">2021 meta-analysis<\/a> found that among those who do, the prevalence of regret is 1%. The report further claimed that many transgender or nonbinary people seek to \u201cdetransition\u201d (return to their sex assigned at birth). The accounts created backstories and personal testimonies to enhance credibility and drive sales through TikTok Shop links. One account, for example, featured videos of an influencer claiming various identities, such as a doctor and former model, to endorse hair growth supplements. A reverse image search, though, showed that the woman was likely generated by deepfake technology. Following publication of the investigation, the accounts identified in the article were removed from TikTok. This tactic reflects a broader trend identified in a 2024 <a href=\"https:\/\/www.jmir.org\/2024\/1\/e60283\/\">study<\/a> published in <em>Journal of Medical Internet Research, <\/em>which concluded that the alternative health community on TikTok is more likely to use emotional storytelling to build trust than conventional health videos, a strategy that these deepfake personas are designed to mimic.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A 2024 KFF <a href=\"https:\/\/www.kff.org\/health-information-and-trust\/poll-finding\/kff-health-misinformation-tracking-poll-health-and-election-issues-on-tiktok\/\">poll<\/a> found that most TikTok users report seeing health-related content on the app, and among these users women are more likely than men to say they\u2019ve seen information or advice about mental health (71% v. 61%) or birth control (41% v. 25%) on the app. In addition, about half of women of reproductive age \u2013 those ages 18 to 49 \u2013 report seeing information or advice on TikTok about prescription birth control (54%) or abortion (48%). While fewer than half (40%) of TikTok users say they trust information about health issues that they see on the app at least \u201csomewhat,\u201d this rises to half among women ages 18-49.
